New Patient Registration
We have an open list and welcome requests fo
r registration from patients living in or moving to the practice area (if you wish to check that you live within the practice area, please check on the Practice boundary section above).
Please download the registration forms below or collect a copy from the surgery and complete.
Then contact us to make an appointment with the secretary to register with us. You will need your NHS number.
Please bring 2 forms of identification with you. We require photographic identification and a letter/Utility bill which has your name and new home address on it.
As it often takes sometime for records to be forwarded from your former practice all patients new to our practice are asked to complete a health questionnaire and have a blood pressure check at reception. This enables us to confirm your medical history and ensure all information is accurate and up to date.
Medical treatment is available from the date of registration. Please contact reception for further information.
Online services
We offer patients the opportunity to order medication which has been authorised for repeat by the GP 24 hours a day, from the convenience of their own home. Simply register at reception,
download the online services application/information and proxy forms if needed below and complete, take to reception who will give you log-in details. (Please bring photographic identification)
For these services use the link on the home page or prescription page once the receptionist has given you the log-in details. Please understand that for security reasons this will only be given in person and you must provide proof of identity.
