New Patient Registration

We have an open list and welcome requests for registration from patients living in or moving to the practice area (if you wish to check that you live within the practice area, please check on the Practice boundary map below).

We are proud to be part of Safe Surgery. You have the right to register and receive treatment from a GP practice without a fixed address, identification, immigration status or NHS Number. The NHS is here to help and keep you safe and well.

To register please either use the link below to process your request online or print a paper copy. You can also pop into Reception where you can collect a printed copy of the form.

Please Note

We do not allow children under 16 to be registered without a parent or guardian also being registered.
The parent or guardian are welcome to register with the child, otherwise the child needs to be registered where either of the guardians are.

It can take up to 10 working days to process your new registration therefore please ensure you have ordered sufficient medication from your current GP surgery if necessary. If you are currently taking any medications and have a Long Term Medical Condition, then please book a medication review as part of your new registration process.

As it often takes sometime for records to be forwarded from your former practice all patients new to our practice are asked to complete a health questionnaire and have a blood pressure check at reception. This enables us to confirm your medical history and ensure all information is accurate and up to date.

Medical treatment is available from the date of registration. Please contact reception for further information.


Online Services

We offer patients the opportunity to order medication which has been authorised for repeat by the GP 24 hours a day, from the convenience of their own home. Simply register at reception,

download the online services application/information and proxy forms if needed below and complete, take to reception who will give you log-in details. (Please bring photographic identification) 

For these services use the link on the home page or prescription page once the receptionist has given you the log-in details. Please understand that for security reasons this will only be given in person and you must provide proof of identity.


eDSM (Enhanced Data Sharing Model) – Sharing Electronic Records

This is to allow the patients full electronic medical record to be shared.

This allows health professionals involved in your care to access your medical records – so they can see what we have and we can see what they have. For example if you are having treatment from the District Nurse team the GP will be able to see that and take into account when planning your treatment.

Your consent to this process has been assumed but you can withhold your consent by telling us. You can also withhold consent on behalf of your children or anyone whom you have Power of Attorney.
